Care Planning Conferences

Shortly following your admission and annually thereafter, the social worker will invite you and your significant others to a Care Conference. This will allow us to get to know you and your likes, dislikes and any issues that require attention.

Care Conferences provide the opportunity for all of us to share information, ask questions and better address your wishes to ensure your stay with us is as positive an experience as possible. Your care plan and current medication list will also be reviewed with you.
